Sword vanished when Crota enraged
My fireteam had just got the third sword on hard crota and the second he became enraged the sword disappeared into thin air that would've been my first time completing hard crota but because the sword vanished we couldn't complete it and we could never get crota down that low again afterwards

Could be lag also, make sure whoever ur swordrunner is, he is hosting fireteam to synchronize crota timer to him/her.
