If you are unable to report in-game, you can do so using the [url=https://www.bungie.net/en/Help/Troubleshoot?oid=13967]form provided here[/url]. At least for me, after clicking "Contact Help" (bottom of page) a form opens up with fillable fields, and a "Submit" button.

Thank you! I don't know why I wasn't able to find that page on my own, but the reporting through Game History is exactly the sort of thing I was looking for. And no worries, I wouldn't post a name on an open forum like that. Thanks again for the link and quick help!
