I just beat Prophecy solo flawless for the first time and did not get the triumph. I got the solo triumph but not flawless both the Bungie destiny website and the dungeon reports say I did not die. I started fresh. the only thing I could think of would be during the first boss fight I think I got teleported due to being pushed off the map, though that could've been the previous run I cannot remember.
