You need to start actually thinking about how new content (weapons, abilities, etc.) will affect the pvp sandbox. It truly seems like you guys just don't care. You need to remember that it's your PVP community that keeps the game somewhat alive during content draughts. PVE players leave and find another game to play when there's no new content, but PVP players will always stay and play, because thats just how people are in general with PVP games. PVP games dont die as fast as PVE games because a player can enjoy PVP as long as they are wanting to improve and get better at the game. You can only get so good at killing AI enemies unttil it becomes absolutely boring.
Right now, D2 PVP has been completely neglected in terms of the (unfun and cheesy) sandbox, and the matchmaking. Fixing the former problem will inherintly help you solve the next. By actually addressing sandbox problems in PVP (namely overpowered ability spam), you will draw more players into PVP (because it will be more enjoyable), and then you can actually impliment some form of lobby balancing because you'll actually have the player count to do so.
Just remember. Trials of Osiris and your PVP community were the only things that kept D1 alive during massive content draughts.
The only thing D2 PVP has going for it to keep any amount of players, is the satisfying gunplay. However, we're losing that because of the ability spam. You addressed this in the past and it actually worked very well in Season of the Lost, but Witch Queen and Void 3.0 (namely bastion and invis) just turned us back around in the opposite direction, and now the same with Solar 3.0 (namely healing nades, radiant, and sunspot healing+lorely). I do not have high hopes for Arc 3.0. You're probably gonna give us a grenade that we can eat and gain wall hacks or some crap.
Remember you have 2 playerbases! And think about how new content will affect BOTH playerbases.
