I just finally got finished with a Solo run of Duality that I also had flawless-ed. I don't have video proof but the best I have is Braytech stats. Right at the end, with Caital having a third of her health, I got reconnected back into my own solo instance, but never to orbit. The final encounter was still in progress too, so I just got back to it and finished the encounter. But I am extremely upset at the fact that that has prevented me from getting any of the solo/flawless triumphs despite ZERO failure.
