I Have the exact same problem Dont initiallize your ps4. I already did it and didnt Work. Im on ps4 pro. I have done everything and still the game freezes all the time. I already disconnected from the Wall to clear caché, reinstalled d2 2 TIMES!!, changed hdcp to 1.4, boost mode off, hdr off, put the game to 1080p, clear database, changed hard drive. And nothing Works, all of my others games Works Fine even in 4k mode with boost mode on The things that I havent tried yet: -Disconnect cmos battery -Change thermal paste (100% sure this is not the problem cause). -change power supply Im almost 100% sure this is a problem with the game and not my console, some people are saying that the actual PATCH its causing memory leaks and thats why D2 freezes/crashes Bungie plz fix this, I cant play like this, I cant even play the campaing.
