I was playing co-op on the final mission, I got pushed back off the right side of the island into the lake and died. When I respawned I was back on the other side of the lake and couldn't go back across the bridge, I had to leave the fireteam and rejoin to get back to the encounter. (I was not teleported into the memory pool area either)
