Sometimes the Cartesian Coordinate fusion rifle fails to fire. This usually happens on the 2nd or 3rd shot, but occasionally the first. Trigger is pulled but nothing happens, no fusion spool up at all. Happens regularly in PvE as are often firing many consecutive shots, but also can happen in PvP leading to your immediate death.
