In the Ruins of Wrath activity, it is possible to jump through a gap between rocks (cliff face facing the Outer Islands where the Barrier Breach and True Sight items activate after the first beacon and an Ager's rock can be found) and fall into the Undercroft without activating the Hive plate to summon Scoroboth, causing the player to become permanently stuck with no way of returning to the surface or continuing the mission as the portal to return to the entrance does not spawn.
