on my last one i didn’t die...where do i see my prophecy runs?

While the triumph shows as completed, you never claimed it, which you needed to do to receive the sparrow. As has been noted, the triumph has been removed from the game, and is now impossible to claim. You will have to hope that when the dungeon returns it will still award the sparrow, and you will have to complete it flawless again.
