Can't see anything you've done wrong. Just make sure you are having all 6 take gaze every time you have anyone take it. If it "Fails to capture Golgoroth's gaze" before all 6 people have taken it at all in the fight then it's a wipe. On the last phase, as long as the gaze is held anywhere within the 6-man chain when Golgoroth dies, you should have it.
