Hello there,  A name reset to default like that means it was detected or reported, reviewed by Bungie and reset to"Redacted####", that indicates that some part of the name broke the Code of Conduct or the Bungie Name Policy, was filtered, and cannot/should not be used again. When that happens, you get the new name cross play message in-game as notice of the name change. If you have no available tokens, then at this time there is nothing that can be done to allow a custom name change or correction, as tokens are not granted upon requests. Stay tuned to the TWiD and official Twitter for updates

Not sure how that plays into this besides a review but they've started mentioning it.
If you'd like to appeal your name change, please visit our Appeal Ban or Restriction Contact Form and make sure to select the reason for the appeal is because your Bungie Name was changed.
https://www.bungie.net/en/Forums/Post/265172235?sort=0&page=0&path=1
