To me there are 3 main problems, I'll get the easy one out the way first:
1) Network
The old P2P model that Bungie's Halo run on wasn't perfect, but this new model just seems to complicate everything and has made it worse. Of course servers would be best but I doubt that will ever happen so I wouldn't mind a switch to a more traditional P2P method. As it stands there is simply too much lag, latency and network manipulation.
2) Balance
Bungie just can't seem to get it right, exclusive/pinnacle weapons aside there is just too much involved for Bungie to give every weapon the same TTK so instead of overall balance we have meta's. Which only ever pleases some of the people some of the time.
3) Gametypes and setups.
Traditionally online games have many PvP options and reduce over time as the playerbase dwindles. D2 started with 2 playlists and both were 4v4. We lost so many gametypes and team sizes from D1 and it really made no sense. Its improved a little but still not enough, in the recent Vidoc Bungie said they want to focus more on PvP in year 3, I hope that means more gametypes, more maps and bigger maps. I can't see how they thought First Light, Skyshock etc weren't successful in D1.
These are what I feel are the biggest issues, you can add to this the matchmaking algorithms and geographical matching but I don't think they would be problems if the above was fixed.
In short, theres just too much BS going on in almost every match. I don't see too much lag and latency but I can understand how its worse for some than others. Even in a good match there are still too many WTF moments. I try to play it, I want those cool crucible pinnacle weapons but after a game or 2 I just feel I'm smashing my head against a brick wall.
