RNG seems to be your main source of creating game time & replay-ability, instead of relying on quality content.
I think if items were very expensive, & had to be earned, it would make obtaining the item way more enjoyable, and you'd have a sense of accomplishment along with earning said item.
So, imo, if items were very expensive, & there were Quests involved, and we knew what we were getting and how to get it, it would be much better than the lazy disrespectful RNG slot machine.
Same goes for weapon and armor perks. Everything should have a path to be earned.
Instead of level 40, how's about level 500, with unlocks along the way?

I think it should be a mix of the two. Take borderlands 2 for example. This may be the best loot shooter ever and they utilize both RNG and mission or area requirements to find certain legendaries. Take the bee shield for example. Everyone knows killing treants in the Forrest is the best place to farm for the bee shield. However, it is still RNG if the bee shield actually dropping from a treant. This leads to farming. Which is a huge and fun part of the borderlands series. Now what it seems you are proposing is I get one bee shield every time I kill treants. Not only would it severely limit game play time, it takes out farming. Which is an integral part of any successful loot shooter.
