I've found that when I'm fighting enemies on stairs with a sword (and sometimes with melee), I'll swing through the enemy and not connect. I'll waste for charges before the sword finally connects with the target. This is after the melee update. This also happens on slants, and it's getting incredibly annoying, especially during PoE41 when every charge is precious.
