I've had the same exact issues. A Bnet moderator told me to clear cache by unplugging my PS4 for 5 min which seems to solve it temporarily but it comes back and appears to be a consistent issue.

Yeah it eventually comes back, and you don’t know until you’re already in a match. This bug is preventing progression for the Crucible-based quests like Claymore’s double kills requirement. Hope it’s fixed soon.

Looks like they added a fix to this in today's update.
