I've been having the same problem on 360. It is a huge relief to hear someone else is having bc after searching and reading I was convinced my disc drive was failing. Only here because I'm a monkey plague victim, but definitely can bump this. My only solution: start box, clear system cache, start destiny and play. If I leave/turn off and come back after that session, it's a no go. Luckily I usually only get to play once a day at most.
