Derealize has a respawn bug right after the puzzle encounter, if you fall off the map before the cp updates. It will spawn behind a kill wall and keep killing you over and over. Also, if you killed the boss during the small white bubble phase after pushing him outside of the white bubble (easy to do with swords), then the symbols for the puzzle won't spawn. Took me 5 runs to get a non-bugged run.
