With the recent changes to the Portal, along with revive system, it has caused an issue in Onslaught. Once you make it to the boss fight part, and if your entire team wipes, it will respawn you back outside the portal where you can try again. However, once you defeat the boss it locks the activity. Chest does not spawn and the exit portal does not appear. It has happened twice so far, and seems to only occur if the entire team wipes while inside the portal. Did not see it on the TWAB under the 'known issues' section so I thought I would list it here. Edit 1: Fixed spelling.
