Mind the Vexplosion triumph not working.
As the title says
I ran Spire of the Watcher without dying today for the first time. I was excited about it, but I never got an achievement for it. I have the confirmation from Charlemagne to prove it (It was all a single run, I never left).

1 RispondiDoes the triumph require a full fire team flawless? Its a shame that it doesn't specify that, but that would explain it.
