Long story short the bells in Duality seem to have a small chance to kill you if you shoot it, no matter where your positioning is. I have experienced the bug 4 times, only on Caitl. However I have heard of, and seen videos of it happening in other encounters as well. This is a very frustrating bug during solo flawless runs or just in general.
