I keep encountering this problem when doing the Warsat PE, The Heoric boss, Shrek, whatever just straight up disappears. Now it’s not a serious issue, but it keeps happening. It can get intensely annoying slashing at an ogre, and the hits me with the Instant Transmission Kamehameha.
