In Gambit, when trying to activate the portals in the Dreaming City themed map, if two people go through at the same time the portal may not trigger so one falls to their death. (If you dont realise and stop holding forward) Also, my teammate died during intital transmat. Maybe because we were running around like crazy and the game tried to teleport two of us to the same position? It was pretty wierd...
