I also did the double reset, have all the dvalins, crucible quest, tested all weapons, and still don't have rank 5.
Seems that the few that have it are hellbent on not telling anyone how they got the extra rep. It's driving me nuts..

Part of the crucible questline gives rep, and i think some of the crucible bounties that unlock after you do the quest give rep

The first week of TTk gave us two sets of gunsmith weapons... Some on Tuesday then on Wednesday on the gunsmith weekly reset
