Found another fragment in the Mausoleum. When exiting the the cabal ship immediately hug the left wall. Keep going tell you go down a small flight of stairs then enter the doorway to your left. Inside there is a small cave on the left wall. Go inside the cave turn left and look up. There's a platform you can jump on. The chest of maggots is also there. I believe this was fragment III
