When any great empire falls, it is never outright destroyed. Instead, it fragments and shatters. That's what happened with the Covenant. In the end, what we call, "Covenant", is dozens of factions lead by warlords. The most powerful of which, is in Halo 4 and lead by Jul 'Mdama. But by the time Halo 5 rolls around, he has lost his power.
