I believe that if you die, you lose times vengeance. As for not having atleast radiance speed grenade recovery that sounds like a bug.

I think my weapon damage was still boosted, actually. Could be wrong. I haven't encountered Radiance not boosting ability recovery times anywhere else, including other places in the vault. This seems to be the only time the issue is apparent. At the very least, I think that "Time's Vengeance" is involved somehow.
