It disappeared on me the quest DYADIC PERFECTION after
I'm done completed the quest A Hundredfold Path

If you haven't already done so, could you kindly verify whether it's possible to retrieve it via the Shaping Slab? Or by changing which Path your Shaping Slab is set to? Also, please be sure to check the quest archive, as it may be possible to reacquire it from there. Please let us know how this goes.
