On the website, you just go to My Legend, Stats, then click the tab that says Crucible and scroll down until you see your Crucible Score (it might be Medal Score). Just below your score, it should display the top medals (medals with high point values) you have earned in the Crucible.

I went there and couldn't find any medals, although I know for a fact I've gotten two before because I've seen them on the app.
