Before the patch the other day, I was on Shadowkeep's 'Beyond' quest, but was unable to progress the story despite completing that mission multiple times. After the patch, 'Beyond' was missing from the map and was replaced by the mission marker for the Shadowkeep mission 'A Mysterious Disturbance'. I completed that mission twice, but was not credited with completion either time. I logged out and back in after some time to notice that all my Shadowkeep related triumphs were completely gone (not showing incomplete, totally missing).
