If I’m not mistaken there’s RNG protection after your 40th clear and you’re guaranteed after that. Note that the shell and ship (from PoH) are tied to the same RNG counter. So if you do 20 altars and get the ship on your first completion of PoH, your RNG protection for the ghost shell resets. You could still randomly get it, of course, but you’d have to do another 40 clears to be ‘guaranteed’ to get it on your next clear.
I got my PoH ship on my 40th clear for my Harbinger... Keep at it, you’ll get it eventually. Good luck, my friend.

You’re guaranteed to get one after 40 clears. Refer to the quote I pulled from DMG above, as that’s the most clear I’ve seen it articulated.
