do you somehow have one open in pursuits maybe? that needs to be completed first or abandoned?

The only pursuit that's relevant to Titan at all is the rat kings crew, would that have anything to do with this?

to follow up, you don't have any other heroic adventures open in pursuits do you? even from other planets?

no, that shouldn't affect it. i do know that on some planets you have to 'buy' the adventure, kind of like picking up a bounty. that shouldn't be the case on titan though or on the weekly flashpoint planet, my map showed the heroic that was available.
