Regarding bullet point number three:
In the new Eris Morn Grimoire card added in AoT, it details her encounter with a awoken male guardian named Asher Mir.
Asher Mir had a missing arm, and had replaced it with vex technology.
Whether he did it for the sake of it, or did it because his arm was missing, remains unknown. But it is noted that his arm was no more.

I'd be willing to bet that the arm is still latently affected by Vex time manipulation, if not controlled by the Vex, and that's why his old arm won't come back. The Vex arm wrote itself into his very being/timeline.

I am aware of that, but I believe it to be an anomaly with the Vex, as this sort of this is not at all normal, and given the condition of the Guardian and Ghost, was likely forced upon the Guardian rather than him making the choice. For this reason, I stand by what I said in the post.

What about if the person had a missing limb long before he passed away in his previous life?

I would argue that because the Ghost would have the DNA pattern of their host, they could revive the new Guardian with their arm. However, due to a lack of evidence for either side of this issue, all we can do is speculate.
