I have the same problem. At first, I thought it was due to the Luck in the Chamber perk too, but got a Vanity to drop with Zen Moment and Firefly instead of Luck in the Chamber. My game will still freeze if Firefly activates at close range. I think it might result from Firefly activating on the final round and the reload sequence plus firefly causes a screen freeze.
