Osiris hired the Sunbreakers to shore up the City's defenses. Zavala proceeded to tell them to f*ck off, after Osiris had already established a contract with them. Zavala in general is the stupidest blueberry in all of Destiny.

Crota accidentally letting the Vex into the Osmium Throne. Lol The Vanguard pressing the assault on the Moon even after discovering the Hive were more dangerous than expected. Great Disaster. Building a Vex gate in the Tower. Having Oryx kill you so you can slip into the Ascendant Realm, then waiting until he [u]hopefully[/u] gets killed. Mara would have died in there if Toland hadn’t just so happened to stumble upon her. And then the plan failed and Mara didn’t even get Oryx’s power. Really a bad idea. That’s what you get, you cheater. Ana Bray faking her death in the middle of Twilight Gap, when humanity as a whole was on the verge of being wiped out. And all to chase after a Warmind that’s “misunderstood” even though it killed hundreds of the heroes that brought humanity together and invented the idea of Guardians protecting humanity just for saying “Hi.’” Rasputin deciding on a whim to go against his protocols and not vaporize Ana when she found the BrayTech Futurescape. TAKE THE SHOT! -blam!-ing with Aunor when she has a train at her disposal.

Notice how 2/3 of his decisions were against Shaxx's advice ( I don't think we ever got Shaxx's opinion on assaulting the Tangled shore) Edit: forgot that he tried keeping the Sunbreakers away from the city during a huge battle (I think it was six fronts?)

Mara Sov suddenly deciding to attack a single Fallen fleet on it's way to Earth and expending the whole stock of surprise manuever then at her command. The results were nice enough, but she could have leveraged such a position into ruling the entire system, hands down.
