Unable to get Moirai
I finished the triumphs to obtain Moirai (complete Forging a Legend I and II, then Legend Forged), but for some reason, I did not get Moirai after claiming the triumphs. It may have happened because I claimed them as a cutscene started, but I do not know how I did not claim Moirai.

You haven't done Commencement on Legendary on your Hunter. Complete the Commencement mission then check the Altar of Relativity, you might need to restart D2 once done as sometimes the Altar doesn't offer it immediately. There's currently a bug where The Invitation on Legendary will check itself and Commencement off on the triumph, but that doesn't actually count as it being completed for the Exotic unlock.
