The way I've been imagining it is you have to get all the way to the end of a dungeon or area before you get loot.

Perhaps, but it seems to work well if there are usable items to be had along the way, like health packs, special grenades, and the occasional coin.

I agree, but I think the major loot such as unique weapons and gear will be at the end. I'm basing this off a story given by one of the developers. I've provided the link; the part I'm referring to is under "Dungeon-Like Raids".

Of course, even in Skyrim the good stuff was always at the end of the tunnel.
