Honestly usually this indicates one of your sticks of RAM is going bad, or at least that has been my past experience with this.

I frequently play a bunch of other games with zero issues that also have transitions though, would it be exclusive to Destiny?

I mean it theoretically could be. It could be that those other games just don't use the volume of RAM that Destiny is using on your computer. I'm just saying what i've seen from experience.
