Wasn't he the Titan on Eris's fireteam? The wizard in the dark beyond mission is verok right?

Yes the wizard is Verok.

So I would assume the dead Titan there is Vell Tarlowe and verok's thrall overwhelmed him when they went into the temple. Verok's thrall would then come back when you "awaken the hive."
