Thanks! I believe it is simply a text labelling error; the quest should say sixth instead of second. When I did the sixth chest it completed.

No, I found a different chest than what the quest prescribes, and that finished the quest :->

Was it the one that spawns in 3 yellow-bar Minotaurs or the one with the Giant Minotaur?

It was the tunnel puzzle chest, the one with the underground maze accessed from Maya's lab and then the left portal.
