Good luck; posted a thread for the same problem clarifying that I'd double checked each chest and rubble pile and added a comment that my inventory wasn't full.
Just got a non-answer that I needed to check everything and a reference to a known bug if your inventory is/was full when doing any of those. They're just copy-pasting answers at this point.

I was stuck on 15. I got 1 from the meatball and was stuck on 16 so i went and deposited my motes in the speakers hall/room for shits n giggles and lo and behold i was missing the travelers mote from the devide in patrol. I was damn sure i had collected it and went back and couldnt see it. spent another day being annoyed and went back again and there it was. to clarify i was in patrol all the time as i heard it doesnt appear during a mission. finally got the damned thing though helped me by pointing out which i was missing... if you cant deposit all 8 free roam motes it will indicate which is missing. if you can deposit all the motes then its an overthrow boss your missing.

My issue was the bit codes; one of the rubble piles didn't give me a bit. I ended up going back with another buddy as they did the cyst and it generated the rubble pile for them; was able to pick it up again in their cyst instance but not mine.

I've already done this as well. Multiple times. It's 100% bugged at the moment. And my inventory isn't full. Also, I was at 14, saw the last rubble pile in the Moth Cave, but was with a teammate. They grabbed it first. Then I grabbed it and I didn't go from 14 to 15 but the triumph activated. So that's where the bug happened. Others have said it happened to them when they were with a teammate as well.
