The steps that I took were beating the main story, completing an anomaly for Shiro-4 along with finishing his entire questline, then Saladin presented me with the option to forge it. When granted the quest, you must first collect several Iron Medallions scattered around the Plaguelands and one on Felwinter's Peak. I used a high range sniper to pick out the symbol for it, it looks eaxctly like the triangle-shaped patrol symbol. From there I followed the rest of the questline, going to Bannerfall and finishing that mission by defusing two large bombs. After that, I had to gather 5 Dormant SIVA Clusters. (Already had 3, used YouTube to find two random ones.) Finally, I had to go to Skyshock to forge the weapon and boom, back to raining death Year One style. Hope this helped. Edit: The quest's name is Beauty in Destruction, for those who do not know. I've also noticed that many people don't seem to know how to locate the Anomaly. In order to find the Anomaly, first get the quest for it from Shiro-4. If he does not present you with the quest, go through his questline until you get it. From there, head to Archon's Keep and look for a Patrol beacon with a Ghost-shaped symbol. Once you have it, follow the patrol instructions until you locate a Fallen Walker along with an Iron Battleaxe. Lay waste to the Walker and scan the environment and you should be rewarded with the Ballerhorn quest.
