As title says, whenever a fireteam member gains the buff, it will not open the third door in the second room, causing a soft lock and not being able to complete the encounter. Edit - apparently this happens whenever the Minotaur is killed at the start of the encounter. I have the video where you can clearly see the boss is bugged and will not move once it enters the first room.
