Red death, I got it as my first exotic when I beat the level 28 PoE for the first time.

Patients and time! Got it from a purple engram back when purples mostly gave you blues.

Armor was No Backup Plans. Wore them for forever thanks to VoG giving me the finger. Weapon was Red Death. Loved it, put it away, now love it again in PvP.

Well my first exotic ever was, Helm of Saint-14, my first weapon was MIDA Multi-Tool, so I dunno which one you mean

Everyone writing weapons, either I am the only one who's first exotic was an armor piece or these guys just have a shitty memory. Either way my first was some armor piece that I cant remember and also got dismantled a while ago because I got the same exotic with better stats.
