After lots of messing around turning off free sync helped with the bad screen tearing but my issue now is some sort of insane fps drops at the beging of any activity.
I'm going to try a different hard drive this evening and just keep trying.

In relation to tearing and input lag I can tell you that even without freesync, you can get a good experience putting Vsync On, and limiting your framerate 2 or 3 frames below your refresh rate. Say you have a 60 Hz monitor Vsync On and FPS Limit to 58, with this, you won't have tearing but you won't have too much input lag either. I read in some articles that Vsync by itself adds like 40ms, but this method I am telling you only adds like 14ms Besides that, I also get more stutter by having the game in fullscreen mode, so I use windowed fullscreen which has less stutter for me

Turns out something is wrong with the SSD. Changed to a hybrid drive I had lying around and it stopped. My theory is a problem with ryzen sleeping cores and the SSD providing the information to fast for the CPU to keep up.
