Another time was when I was playing my level 12 at the time warlock on Venus. I noticed some guys going into the vault of glass, and I was like "lol, I'll follow them!" So I did, and I got partway down into the vault, not quite into the first room, when I decided this was a very bad idea. I turned around to leave and the door shut.
