I still want to know who "poisoned" Robin's mask and made him start seeing Slade.

It was Slade himself who poisoned his own mask. Robin took his mask as a piece of memorabilia and Slade coated the mask in hallucinogenic chemicals that caused him to see Slade, as Slade knew that Robin was obsessed with him from the start. He knew that the memories would haunt him and thus he planned accordingly.
