Excision.
Twice joined an LFG to do this GM and both times when the Fireteam leader starts the mission, the whole thing hangs in space, then with a black screen for ages then one by one a message pops up saying x has left your team. Everyone then ports into the instance on their own.
Wasted 45 minutes on this tonight.
