Ikora's Tasks 4/5
Completed her quests up until the last one, all other side quests complete but fifth one is no where on the map screen.

I'm also having the same issue, finished campaign on legendary and fabled and checked abandoned quests kiosk and nothing. Just says 4/5 on tasks for the triumph.
