There is one in the Hall of Souls up high that requires the jumping puzzle to spawn. Took me a while to get an instance where that jumping puzzle was present. If it doesn't spawn, you might easily miss that...I didn't use a guide...just hunted high and low...but try another guide...maybe the different perspective will help. If you have been to every location (check, double-check), perhaps try a different character.
